[  ] 2 - Drop a note or call friends and give them your new address and phone numbers

Financial, Insurance, & Legal

[  ] 1 - Submit address change and order new checks from bank
[  ] 2 - Submit address change to all financial savings and lending institutions
[  ] 3 - Submit address change to insurance company
[  ] 4 - Submit address change license bureau (drivers license)
[  ] 5 - Submit address change for voter's registration
[  ] 6 - Submit address changes to credit card companies and other creditors

Miscellaneous

[  ] 1 - Submit address change for newspaper service, bottled water service, etc.

          representative and advise total number of boxes that you require
          to be moved
